Photochemotherapy of Recalcitrant Pustulosis of Palms and Soles

Abstract
Treatment of recalcitrant pustulosis of palms and soles presents a most difficult problem. Mizuno (1976), Morison (1978) and Murray (1979) reported benefit of photochemotherapy for this disease. The present study was undertaken to evaluate the therapeutic effectiveness of topical photochemotherapy ¢¥combining topical application of 0. 1% 8-methoxypsoralen ointment and UVA exposure using high pressure mercury lamp, in comparison with the topical cortcosteroid cream applied on the opposite control site. A total of 6 patients (4 males and 2 females) with recalcitrant pustulosis of palms and soles were selected at the department of dermatology, National Medical Center from October 1979 to September 1980.
The results were as follows.
1. In the site of photochemotherapy, 3 cases were cleared over 90% of the lesions (Response 1), mean number of PUVA therapy was 11.7. During 5 months¢¥ follow-up period on., case (case 5) remained free of lesions. PUVA treated site was responded more rapidly than the control topical corticosteroid applied site.
2. Side effects of therapy were very mild and temporary, included only transient pruritus.
3. Comparison of pre- and post- PUVA therapy in biopsy specimens by light microscopy revealed histologic improvement evidenced by diminished acanthosis, parakeratosis and disappearance of intraepidermal pustule.
